About the E4 Network PCN
The E4 Network is a Primary Care Network (PCN) established in 2019, comprising five GP practices in Chingford:
- Handsworth Medical Practice
- Old Church Surgery
- The Ridgeway Surgery
- Churchill Healthcare
- Chingford Medical Practice
Together, we provide care for over 53,000 patients in Chingford and are committed to enhancing primary care services for the community.
What is a Primary Care Network (PCN)?
A Primary Care Network (PCN) is a group of GP practices working together with other healthcare providers to improve patient care in a local community. PCNs were introduced in 2019 as part of the NHS Long-Term Plan to enhance collaboration and deliver more integrated services.
This helps to:
- Improve access to healthcare services
- Offer a wider range of specialist roles, such as clinical pharmacists, physiotherapists, care coordinators, general practice assistants and social prescribers
- Enhance preventative care and support for long-term conditions
- Reduce pressure on GP surgeries by streamlining services
Why Are PCNs Important?
By working together, PCNs can provide personalised, proactive healthcare while ensuring patients receive the right care at the right time. This collaboration helps GP practices to:
- Provide more appointments and extended services
- Focus on preventative medicine and long-term conditions
- Work closely with hospitals, community services, mental health teams, and social care providers
