Care Home Management
E4 Primary Care Network (PCN) will aim to enhance the health and well-being of the residents living in the Nursing & Care Homes aligned to our GP Practices by providing clinical input, personalised care and developing support plans to those who are vulnerable and severely frail. These homes are:
- St Ives Lodge
- The Spinney
- Gracewell
- St Francis
- Three Willows
- Heathlands Nursing Home
The service’s we will provide to our registered Care/Nursing Homes include, but not limited to, the following:
- Structured Medication Review
- Falls Prevention
- Case based discussions at MDT
- Weekly care home round
- Support for patients/residents as they enter the care home
- Personalised care plans with the patient and/or carer for patients of admission to home and readmission following a hospital episode – developing existing assessments to reflect patient’s goals.
